Principles of Body Mechanics Every Nurse Should Know


OSHA's Safe and Sound Week is recognized as August 10-16. This year, we are focusing on safe lifting and body mechanics.

Safe patient handling in nursing relies on two critical components: proper body mechanics and, most importantly, the use of assistive devices and technology. Manual lifting alone, even with perfect form, is not sufficient to prevent injuries due to the high forces on the spine. Safe lifting for nurses involves using proper body mechanics such as maintaining a wide base of support, keeping your back straight, and lifting with your legs, not your back. Always keep the patient or object close to your body and pivot your feet instead of twisting your spine. When possible, use assistive devices like mechanical lifts and gait belts to prevent injury, and ask for help with heavy lifts.

Principles for safe lifting and body mechanics
  • Maintain a Wide Base of Support: Stand with your feet shoulder-width apart, with one foot slightly in front of the other to improve balance and stability.
  • Use Your Legs, Not Your Back: Bend at your hips and knees to squat down to the level of the object or patient, keeping your back straight and neutral. Engage your powerful leg and core muscles to lift, rather than straining your back.
  • Keep the Load Close: Hold the patient or object as close to your body's center of gravity as possible. This minimizes leverage on your spine and reduces strain.
  • Avoid Twisting and Reaching: Never twist at the waist while lifting or holding a load. Instead, pivot your entire body by moving your feet in the direction you want to turn. Avoid overreaching, leaning, or stretching.
  • Keep your back straight: Avoid bending or twisting at the waist. Keep your spine in a neutral, natural alignment.
  • Use assistive devices: Utilize mechanical lifts, gait belts, and slide sheets whenever possible to reduce physical strain.
  • Ask for help: If a lift is too heavy or you feel unsure, ask a colleague for assistance.
  • Plan the Move and Communicate: Before any transfer, plan your path, clear obstacles, and communicate with the patient and any assisting colleagues (e.g., counting "1-2-3") to ensure smooth, coordinated movement.
  • Work at an Appropriate Height: Adjust the height of beds and stretchers to waist level when providing care and hip level when moving a patient to avoid excessive bending.
Additional tips
  • Push instead of pull: Whenever possible, push objects to engage larger muscles and reduce strain.
  • Take care of yourself: Wear comfortable, non-slip shoes and take breaks to stretch and move around to prevent fatigue.

Prioritize Assistive Devices and Equipment

The American Nurses Association (ANA) and OSHA advocate for the elimination of all manual lifting and prioritize the use of safe patient handling and mobility (SPHM) technology.

  • Assess the Patient: Always assess the patient's ability to assist, their weight-bearing capability, and cognitive status before initiating a move. Use a standardized assessment tool to determine the appropriate equipment needed.
  • Utilize Mechanical Lifts: For fully dependent patients, use ceiling-mounted lifts or portable floor lifts. These devices significantly reduce spinal stress and physical exertion.
  • Use Transfer Aids:
    • Gait belts/transfer belts provide a secure grip when assisting a patient to stand or walk.
    • Slide boards/friction-reducing sheets help with lateral transfers (e.g., bed to stretcher) by reducing the force needed to move the patient.
    • Height-adjustable beds/stretchers minimize the need for manual lifting to achieve a safe working height.
  • Know Your Limits and Ask for Help: The National Institute for Occupational Safety and Health (NIOSH) recommends lifting no more than 35 pounds of a patient's weight manually under optimal conditions. If a patient is too heavy or the situation is unsafe, always ask for help from colleagues or use equipment.

Implementing a culture of safety, where equipment use is standard practice and staff are properly trained, is crucial for preventing injuries.
